  • The round was led by HealthX Ventures with co-investment by WISC Partners
  • In conjunction with the funding, Mark Bakken, Managing Partner of HealthX Ventures, is joining the company’s Board of Directors
  • The company intends to use the funds to accelerate product innovation and expansion into new markets, continuing focus on client service and platform usability
  • The company provides a digital care management platform, called ConnectedCareTM, which serves 21 healthcare systems in 16 states
  • ConnectedCareTM increases patients engagement and satisfaction by providing actionable and information in real time throughout the care journey
